Download & Login

Step 1: In the email we just sent you, there is a verification link. Click on this to activate your account. Step 2: Open the email we sent you on your mobile phone, click the download link for Apple or Android and download the Digital Forms app. Step 3: Login with the username and password you created.

You might need to check your spam folder if the email hasn't arrived in your inbox.

Make a Submission

Step 1: Select Forms from the dashboard. Step 2: Tap any form and then select Create. Step 3: Fill out the form and press Submit.

View the Submission

Step 1: Login to the web portal (you can click the Finish button below to open in a new tab and login to the web portal). Step 2: Select Submissions from the Management menu. Step 3: View or print the submitted form either online or using the built-in PDF export wizard.
